Please read the following Terms and Conditions (the "Terms") carefully before ordering the 25-Hydroxyvitamin D Test Kit (the "Test Kit") and related materials and health information (the "Test Kit" and the related materials and health information are collectively referred to as the Test Kit Package"). 1.The Test Kit Package

By requesting to purchase the Test Kit Package, you understand and agree that you have not entered into a physician-patient relationship with Natural Products Corporation (NPC), the owners of VitaminD3World and that NPC or any of its employees is acting as your physician. You understand and agree that the Test Kit may be used to collect and submit a blood droplet sample to a laboratory that will analyze the sample and report your 25-hydroxyvitamin D level directly back to you for general lifestyle, nutritional, and educational purposes. The Test Kit and analytical test results as well as the Test Kit Package are not offered for the diagnosis, cure, mitigation, treatment, or prevention of any disease or disorder. Please consult your physician and health care providers with regard to matters pertaining to your health. The Test Kit Package is not intended as medical advice. Please consult your health care professional with regard to matters pertaining to your health.

The total charge for the Test Kit Package is $65 per kit. The clinical laboratory service to be performed on the blood spot samples you may submit using the Test Kit is a 25-hydroxyvitamin D blood analysis. ZRT Laboratory, located at 8605 SW Creekside Pl., Beaverton, Oregon 97008, will perform this clinical laboratory service.

You may obtain the 25-hydroxyvitamin D test yourself at another licensed clinical laboratory of your choice.

If you have questions about your own health and medical condition or about taking the 25-hydroxyvitamin D test you should consult your physician. New York State health law prohibits the testing of specimens collected in or mailed from New York and prohibits the transmission of data from the laboratory to New York physicians or residents. Therefore, we are unable to process orders from New York State or California.

The Test Kit Package and information provided on the website is designed to support, not replace, the relationship that exists between a visitor to this web site (the "Site") and his or her health practitioner.
